- Includes 3 days of pre-hospitalization and 15 days of post-hospitalization expenses.

Who are Eligible for PM-JAY?
The scheme primarily targets economically weaker sections identified through the Socio-Economic Caste Census (SECC).
Rural Eligibility Criteria
Families meeting at least one of the following conditions are eligible:
- Only one room with kutcha walls and roof
- No adult member between 16 and 59 years
- No adult male member between 16 and 59 years
- Disabled member with no able-bodied adult
- SC/ST households
- Landless households dependent on manual labour
Automatically Included Categories
- Homeless families
- Destitute or people living on alms
- Manual scavenger families
- Primitive tribal groups
- Released bonded labourers
Urban Eligibility Criteria
The scheme includes workers from several occupational categories such as:
- Ragpickers
- Domestic workers
- Street vendors
- Construction workers
- Drivers and transport workers
- Electricians and mechanics
- Tailors and artisans
- Sanitation workers
- Delivery assistants and helpers

Who is Excluded from PM-JAY?
The following groups are generally not eligible:
- Government employees
- Families earning over ₹10,000 per month
- Owners of vehicles
- Owners of mechanized farming equipment
- Families owning refrigerators or landlines
- Households with large agricultural landholdings
- Families living in well-constructed houses

Applicants may need:
- Aadhaar Card
- PAN Card or identity proof
- Address proof
- Mobile number and email ID
- Income certificate
- Caste certificate
- Family status proof
